Fees Menu is blank

In Tax Processors Plus Desktop, if you go to the Fees Menu and it is totally blank with no forms showing, a database needs to be repaired:

From the Quick Menu of Tax Processors Plus Desktop select:
- Utilities
- Additional Utilities
- Repair FormInfo - You may be warned that doing this will reset all fees to $0 and ask if you wish to continue. Answer YES.
